PRN Medications & Follow-Up
Navigating the PRN order workflow, documenting indications and pre-assessments, and completing required clinical effectiveness follow-up.
1. The PRN Clinical & Software Flow
PRN administration requires two core stages: (1) assessing the resident and documenting the ordered indication and relevant pre-administration assessment findings, and (2) re-assessing the resident after clinical onset and documenting effectiveness.
2. The Three-Lane PRN Administration Model

- Navigate:
eMAR ➔ Resident list ➔ Resident ➔ PRN. - Select the active PRN order, complete required supporting documentation fields per order/facility setup, and record administration.
- For follow-up: Navigate to
PRN ➔ Follow Up. - Complete follow-up results, required supplementary documentation, and progress note if required; then click Record/Save.
🩺 AT THE BEDSIDE
- Verify valid ordered indication and confirm time interval since prior dose to avoid frequency violations.
- Assess baseline symptoms prior to administration per the ordered indication.
- Re-assess the resident at the clinically appropriate timeframe after administration based on medication, route, condition, and facility policy.
📝 DOCUMENTATION
- Document the ordered indication and relevant pre-administration assessment findings.
- Document post-administration effectiveness and resident response following meaningful reassessment.
- If symptoms persist or worsen, document clinical follow-up, non-pharmacologic interventions, or provider notification.
⚠️ The Premature Follow-Up Hazard
Do not document effectiveness before a meaningful reassessment has actually occurred. Reassessment timing depends on the medication, route, resident condition, and facility policy. Always conduct a physical evaluation of the resident's response prior to documenting the follow-up.
- Document the ordered indication and relevant pre-administration assessment findings.
- Document post-administration effectiveness following actual physical reassessment.
- Complete required PRN follow-up documentation according to the order, resident response, and facility workflow.
