Writing Surveyor-Proof Progress Notes
Documenting objective findings, skilled nursing interventions, and resident responses in the PointClickCare progress notes module.
1. The Clinical Purpose of Progress Notes
Progress notes in PointClickCare form part of the resident's legal health record. They capture focused assessments, clinical responses to care, practitioner communications, and changes from baseline.
2. The Three-Lane Progress Note Model
🖥️ IN PCC (HISTORICAL UI)
- Navigate:
Clinical ➔ Resident ➔ Prog Notes ➔ New. - Select the applicable Note Type configured by your facility (e.g., communication, infection, behavior, or shift summary).
- Link to active Care Plan items or Tasks if applicable in your configuration.
- Complete the entry, verify effective date/time, and click Sign.
🩺 AT THE BEDSIDE
- Perform focused physical assessment (vitals, surgical site, breath sounds, mental status).
- Verify tolerance of specialized care (wound VACs, enteral feeds, oxygen therapy).
- Check direct-care observations (mobility, skin integrity, meal intake).
📝 DOCUMENTATION
- Document specific, objective observations (*"Lungs clear bilaterally, surgical dressing dry and intact, ate 75% of meal"*).
- Attribute reported information directly (*"CNA reported resident unsteady at 14:00; nurse evaluated..."*).
- Note that a progress note does not replace required incident forms or eINTERACT change-in-condition evaluations.
⚠️ The Generic Boilerplate Trap
Copying repetitive generic phrases (*"Resident resting comfortably, no complaints voiced"*) fails to capture changes from baseline and provides no clinical insight if a resident declines later. Document specific clinical observations and interventions performed.
- Document objective, measurable clinical findings (vitals, breath sounds, wound/skin status).
- Document nursing interventions delivered and the resident's response.
- Attribute secondhand information directly to the reporter.
- Ensure required specialized forms (flowsheets, incident records, COC assessments) are completed in addition to narrative notes.
