Order & Policy Reminder: Specific hold parameters (e.g. blood pressure or heart rate cutoffs) are individualized physician orders and facility protocols. Automatic system warnings and parameter thresholds depend on facility configuration. Always verify the active order in the resident chart.

1. The Supplementary Documentation Workflow

When an order requires supporting clinical data (such as pulse or blood pressure), PointClickCare eMAR can prompt for Supplementary Documentation. You record the required clinical finding before completing administration.

2. The Three-Lane Parameter Workflow

Open the scheduled order in eMAR to access Supplementary Documentation.
Enter the freshly measured vital sign value.
If the value is out of ordered parameters, follow facility workflow for documenting held or omitted doses.
Select the applicable facility-configured outcome code if the dose is withheld.

🩺 AT THE BEDSIDE

Measure the assessment or vital sign required by the resident's active order.
Compare the bedside reading against the active physician order parameters.
If out of range, withhold the medication and assess for clinical symptoms (e.g. dizziness, bradycardia, hypotension).

📝 DOCUMENTATION

Record the exact vital sign obtained in the EHR documentation prompt.
If a dose is held, document the outcome and notify the provider per order directions.
Do not select "Use Last Recorded Value" if the order or clinical situation requires current pre-pass vitals.

⚠️ Critical Warning: The Stale Vitals Trap

Historical PointClickCare interfaces may allow selecting "Use Last Recorded Value." Relying on older vitals from hours earlier to administer a vitals-dependent medication or insulin is unsafe. Always obtain fresh, relevant objective data when required by the order.

When a medication is held per order parameters, the exact reading is charted with the hold documentation.
